The health and safety of everyone going to softball diamonds is the No. 1 priority for NASC Softball and the City of Oshawa.
The City of Oshawa's screening tool is a basic screening checklist and is not meant to take the place of medical advice, diagnosis or treatment.
This form will help the city and NASC Softball know if people need to be contacted about potential exposure to COVID-19.
